Nearly Three in Ten Treated Diabetic Cats Reached Remission — and Remission Meant a Better Life

In a survey of 477 insured Swedish cats, 29% of treated cats came off insulin, and remission was linked to better owner-rated quality of life.

Plain-English summary

A diabetes diagnosis in a cat sounds like a life sentence of twice-daily injections. For a substantial minority of cats, it is not.

In this study of 477 Swedish cats with diabetes, 29% of the treated cats reached remission — the point at which insulin is no longer needed. Cats that reached remission also had better owner-rated quality of life than those that did not.

That combination is what makes the finding worth knowing: remission is not just a convenience for the household. It appears to be better for the cat.

What the study found

The researchers identified cats diagnosed with diabetes mellitus and insured by a Swedish insurance company between 2009 and 2013, and sent a questionnaire to 1,369 owners. The response rate was 35%, leaving 477 questionnaires for analysis. Questions covered the cat, its treatment, the owner's perceptions of the disease and its treatment, and disease outcome.

The remission rate among treated cats was 29% — 118 of 405.

Feeding a commercially available wet diet was associated with remission (OR 3.16, 95% CI 1.27–8.12) and with remission without relapse (OR 14.8, 95% CI 2.25–153.8).

Remission was associated with a better quality of life for the cat.

The authors conclude that the diet association strengthens the role of diet in treating feline diabetes, and that linking remission to better quality of life emphasises remission as a goal of disease management.

What this study cannot tell you

Start with that second odds ratio. A confidence interval running from 2.25 to 153.8 is not a precise estimate of anything. It tells you the association is unlikely to be zero and essentially nothing about its size. Quoting "14.8 times more likely" without the interval would misrepresent this study, and the interval is wide enough that the honest summary is: direction suggested, magnitude unknown.

The design carries the usual weight of a retrospective owner survey. A 35% response rate leaves substantial room for response bias — owners whose cats did well, or who remained engaged with the illness, may be over-represented relative to those whose cats died early or who found the treatment overwhelming. Everything here is owner-recalled, sometimes years after the events, without clinical verification.

The diet finding in particular is an association within an observational study. Cats fed wet food may differ systematically from cats fed dry food in ways the study did not measure — owner attentiveness, veterinary engagement, how early the diagnosis came. None of that is separable here.

Finally, this is a Swedish insured-cat population between 2009 and 2013. Insurance changes who presents and how they are treated, and treatment protocols for feline diabetes have moved since.

What pet owners should track

For a cat already diagnosed, the record that matters is the one your veterinary team asks for — and the pieces that are easy to lose between visits:

  • Weight, dated, on the same scale — weight change is central to both diabetes management and remission
  • Water intake and litter box output, if your vet asks you to monitor them
  • Any home blood glucose readings, with the time of day and the time relative to the insulin dose
  • The insulin dose in use and the exact date of every change
  • What your cat is actually eating, including treats and any food from other household members
  • How your cat seems in itself — activity, coat, interaction — since quality of life was a real outcome in this study

For a cat not yet diagnosed, the general signs owners can notice are increased drinking, increased urination, and weight loss. These are not specific to diabetes and are a reason to see a veterinarian rather than to conclude anything.

Do not change diet, insulin dose, or feeding schedule on your own. In a cat on insulin these interact directly, and getting it wrong is dangerous.

Can a diabetic cat really stop needing insulin?

It happens. Among treated cats in this study, 29% (118 of 405) achieved remission, meaning exogenous insulin was no longer needed. This is a feature of feline diabetes specifically — it is not something expected in dogs.

Should I switch my cat to wet food?

Not on your own. This study did find an association between a commercially available wet diet and remission, but diet and insulin dose interact directly, and changing food without veterinary supervision can be dangerous for a cat on insulin. Take the finding to your vet as a question, never as a plan.

The odds ratio for remission without relapse was 14.8. Isn't that enormous?

The confidence interval runs from 2.25 to 153.8, which is one of the widest you will see reported. That range tells you the true effect could be modest or extreme — the data cannot narrow it. Treat the direction as interesting and the magnitude as unknown.

Does remission last?

Not always — the study reports remission and remission-without-relapse as separate outcomes, which means relapse occurred. It does not report a relapse rate in a form that can be quoted here.

How strong is this evidence?

Limited. It is a retrospective questionnaire sent to owners of insured cats, with a 35% response rate. Owners who respond to a survey about a chronic illness their cat had are unlikely to be a random subset, and everything reported is owner-recalled rather than clinically verified.
